Welcome to the Attendance Office
Our office is located at the front of the building, facing West Davis St. We are available from 7:00 am to 3:00 pm, Monday - Friday. Our contact information is as follows:
State law requires that students be in attendance at least 90% of the days the class is offered (excused and unexcused absences combined). Please refer to the CISD Secondary Student Handbook and the CHS Handbook Supplement for more complete information about the policies listed here.
You may contact us to let us know a student will be absent for the day, if a student needs to be released before the end of school, and for daily attendance information.
If your student is going to be absent from school for more than one day, principal approval is required. Please come to the attendance office to pick up the form that must be filled out.
If your student is absent from school, please send a note with the following information
- Student Name and ID number
- Date of absence
- Reason for absence
- Parent name, signature AND phone number
If your student must arrive late to school, please send a note or come inside to the attendance office to sign the student in. Without any information from the parent, the absence will be unexcused.
The best way to arrange early pick-up for your child is to send a note on the morning of pick-up. The student will bring this note to the attendance office and a “Permit to Leave School” will be issued. The student will show the permit to his/her teacher when it is time to leave, and then sign out at the attendance office.
You may also call the attendance office, but this requires disrupting the class to notify the student and teacher. We try to avoid this whenever possible. We are unable to call students out of class after 2:15 pm. You are welcome to text your student to meet you at the front when the bell rings at 2:35.
Proper ID is required to pick up any student or to inquire about a student's attendance record. If you call, please understand that we need to ask a few questions to make sure you are the guardian of the student. We recommend that you know your child's student ID number.
Please email the attendance office and give the following information: student's name, date(s) of absence, reason for absence, parent/guardian name/signature, and contact phone numbers. You may also call or send a note.
Please remind your student to use the clinic if he or she is not feeling well. Students in the clinic are not considered absent. In the event of a doctor's appointment, a note from both the health care provider and the parent must be submitted to the campus.
The Health Care Provider form allows the Attendance Office to change an absence to a presence if the child has been present for part of the day. A doctor's note must also be included. Please check your student in and out through the attendance office. This provides a record of when the student leaves for the appointment or returns, and confirms the student was present for part of the day. [Link]Click here to access the form.
The Religious Holy Day form allows the attendance office to change the unexcused absence into a presence for purposes of observing the student's religious holiday. A parent note or signature on the form is required. Click here to access the form.
(The Student Pick-Up Information Update form can only be filled out by the student's guardian. Proper identification is required. This form allows someone other than the guardian to pick up a student from school. It remains in force until the guardian changes it in writing.
The Parent Request for Excused Absence form must be submitted in advance to the appropriate assistant principal. Approval will be based on the student’s attendance record, academic standing, and compliance with campus procedures. A maximum of five pre-approved absences per school year are allowed. Pre-approved absences are counted when determining if a student has attended 90% of class days. Make- up assignments are provided for absences approved in advance. Pre-approved absences do count against final exam exemption privileges. (link here)
